The referral pathway

Four steps from referral to first dose.

The pharmacy team holds the operational pathway. You retain prescribing authority and clinical oversight.

  1. Send the referral

    Use the structured form below or fax/email a standard referral. Mark "pharmacotherapy" and note current SafeScript status.

  2. Capacity confirmed in 2 business hours

    The dosing pharmacist contacts your clinic during dosing hours to confirm capacity and timing.

  3. Patient attends for first dose

    ID, Medicare and current prescription details are checked. Pickups are coordinated with the prescriber.

  4. Ongoing review

    Stable dispensing with periodic SafeScript checks and clinical handover support when a transfer is required.

What we need from you

Referral criteria.

  • Current valid prescription for opioid dependence pharmacotherapy (oral or long-acting injectable formulations).
  • SafeScript checked at the prescribing visit, please document the date.
  • Patient ID, Medicare and DVA (where relevant) details available at first attendance.
  • Clear handover note where the patient is transferring from another pharmacy or interstate.
  • Photo ID for first attendance, passport, drivers licence or proof of age card.
