Takeaway doses. The Department of Health's takeaway-dose appropriateness checklist is used jointly with the prescriber for every takeaway decision, with the decision documented.
Controlled-drug discipline. Prescription pharmacotherapy medicines are entered in the S8 register at the time of the transaction, recorded at least daily, per the Drugs, Poisons and Controlled Substances Regulations 2017. S8 storage, staff-access controls and a non-public dosing/administration area are in place.
SOPs. Maintained and reviewed on a defined cycle for: dosing, takeaway risk assessment, transfers and visiting, long-acting injectable pharmacotherapy administration, naloxone supply and prescriber liaison.
